Please provide a completed referral slip, to the patient and if possible, and email a copy to our office as soon as possible along with any necessary x-rays!

We will do our best to help your patient in a kind and compassionate manner. We can provide treatment estimates and will process any insurance we can.

We are currently in-network with Delta Dental (Premier and PPO/EPO) we are out-of-network with all other insurance and are happy to submit claims to plans with out-of-network benefits.

We also offer CareCredit.
