Your appointment is scheduled as stated. To complete your paperwork online, via a secure portal, prior to your appointment please visit this page > [ click here ]. There are 4 pages listed on the left of the screen, after completing each one click Next to access the next page – after the 4th page click Submit. Look for a confirmation email that the information was submitted successfully.

If you cannot complete the forms online, for any reason, please arrive 20 minutes prior to your appointment time to complete the forms in the office. Remember to bring a list of your medications. If you do complete the online registration, Thank You! Please arrive 5 minutes prior to your appointment time to sign consent forms. If you are running late please call us to confirm if we can still see you for your appointment. We have a 15-minute cut off from the time you are expected to arrive for your apt.

Payment is due at the time of service.
You should have been quoted based on the referral. If not, or you have any questions, please call the office.

NOTE: If you require medication placement then we will need to schedule a second appointment and it will be an additional cost ($305). We can also provide N2O (laughing gas) for an additional fee of $140 per appointment, which is optional and usually not covered by insurance.

Costs: This estimate is based on the referral provided by your dentist for endodontic treatment. When provided dental insurance is used to determine your out-of-pocket costs. The estimate may be altered later based on additional information not available at the time of the estimate such as frequency limits, deductible and fee schedules. It can also be altered based on actual treatment required that was not anticipated. Once insurance payments clear you may be responsible for a balance due based on the insurance payment.
